HOW LIFE COORDINATED® WORKS
LIFE COORDINATED® is a life-changing membership program that provides access to a personal care advisor who works with you to coordinate the services you are seeking, provide key guidance and help you in the event that there are bumps along the way.
Join
You can sign up as a member for yourself or for someone else. Simply specify the primary account holder in the set up process and designate others that are authorized to receive information such as a caregiver.
Join
For example, caregivers may sign up someone that they are caring for and notate that they are the Caregiver for care coordination.
Membership Level
Select the level of service you are seeking or call to discuss. You can also buy discounted packages of products and services including meals and home modifications such as grab bars.

How do you know if LIFE COORDINATED® is right for you?
Perhaps you want to be more independent and rely less on family and friends, or you’ve had a traumatic injury, or maybe you are planning ahead and need help with universal design and in-home modifications. There are so many reasons to join.
If you are still uncertain, below are some of the most common reasons individuals join LIFE COORDINATED®:
Help with home modifications to “thrive In place” outside of an assisted living or nursing home. As aging in place experts, we design, price and coordinate your in-home modification with our quality partners to ensure the work is done right. This means peace of mind for you.
Pack up the guilt. Caregivers and adult children need help with caregiving - it's exhausting. Yes, you are super human, however you can only be spread so thin. Our team uses LIFE COORDINATED® for our families. This means you can rest assured we are focused on quality.
Independence. You want to remain on your own and don't want to burden your family and friends. Of course your loved ones will help, however who wants to ask? If this has crossed your mind LIFE COORDINATED® is for you. This means independence and the help you want.

Members-Only Services and Discounts
As a LIFE COORDINATED® member you have nationwide access to high quality providers within The CENTIPEDE Health Network. Contact your Care Advisor for help or submit a request for assistance. You will need an active LIFE COORDINATED® membership to access in-network providers. Below is just a sample of the services available to you.
If other services are needed, let us know and we’ll try to help arrange a special deal where possible. Also, based upon the state you’re located in there may be some variation in service descriptions.
Universal Design Services
The LIFE COORDINATED® team has achieved Certified Aging in Place Specialization CAPS® certification which is a designation by the National Association of Home Builders (NAHB). We provide “universal” design recommendations for in-home modifications, then bid the jobs to meet the specifications.
For larger jobs, we can ensure that the correct permits and code standards are met. We also have certified Project Management Professionals (PMP) and Construction Management/Contractors on our team.
Care Navigation
Care Advisors provide personalized assistance in selecting and connecting the right services for LIFE COORDINATED® members. Care Advisors will also take the time to understand if you are eligible for covered services through your insurance, your employer and other programs through the Veteran’s Administration and HUD.
LIFE COORDINATED® is not a PAID referral service; providers CANNOT PAY for referrals unlike other services that promise free navigation and assistance.
Assistive Technology
Assistive Technology (AT) is defined as any item, piece of equipment, software program, or product system that is used to increase, maintain, or improve the functional capabilities of persons with disabilities. Mobility aids, such as wheelchairs, scooters, walkers, canes, crutches, prosthetic devices, and orthotic devices.
Hearing aids, Cognitive aids, including computer or electrical assistive devices, ramps. Some insurance will pay for AT, however sometimes AT must be paid out of pocket.
Private Duty Nursing
Private Duty Nursing (PDN) helps keep someone safe and comfortable in their own home and is paid “privately” to the agency or caregiver. PDN can include meal preparation, bathing and grooming, assistance with navigation around the home, light housekeeping, simple household chores, safety supervision, medication reminders, companionship, and transportation as needed.
A PDN caregiver may also help with doctors appointments. PDN is most commonly paid out of pocket.
DME and Custom DME
Durable medical equipment (DME) is defined as prefabricated medical equipment such as beds, toilets, basic wheelchairs, walkers, CPAP machines.
Custom DME includes vehicle modifications or other items that are specifically customized to the user like motorized or standing wheelchairs.
Home Modifications
“Thriving in place” is a dream for many and may require some home redesign to make your home more accessible. Universal Design standards are focused on making a home more functional and inclusive for all family members.
The LIFE COORDINATED® team will help with universal design and pricing to help you get what you need.
Personal Care Services
Personal Care services are provided by a trained Personal Care Worker or an aide who assists clients with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
ADLS: 6 basic self-care tasks: Bathing, Dressing, Feeding, Transferring, Continence, Toileting.
IADLS: 8 basic tasks: Housekeeping, Telephone & Technology, Shopping, Food Preparation – being able to plan and prepare meals, Laundry, Transportation, Medications, Finances.
Home Delivered Meals
Home delivered meals cover a wide array of choices including meals-on- wheels and pre-packaged meals shipped to your door.
Medically oriented meals are appropriate if someone does not cook or needs help with specific diets such as gluten-free, low sodium and heart healthy. Meals plans are generally designed to maintain or improve health or address clinical issues. Pre-packaged and ready to heat meals are delivered door to door via FedEx or UPS.
Other fresh food meal options are available for those who want to cook but need help with shopping.
PERS (Personal Emergency Response)
PERS also known as “medical alert” services may include electronic remote monitoring for falls, a 24/7 help line, medication monitoring and check-ins.
Home Health
Home health is generally ordered by a doctor and is defined as skilled care, such as nursing services, physical therapy, occupational or speech therapy, home health aides, a dietary/nutrition specialist, and or a medical social worker.
Respite Care Services
Respite care services are designed to give parents or caregivers a break from caregiving and can occur in-home or out of home such as “vacation respite”.
Pest Management
Pest management is more than extermination. Good pest management determines “why” pests are entering your home and addresses the cause.

Grocery Delivery
Help with grocery orders and delivery services.
Disposable Medical Supplies
Disposable medical supplies, such as blood sugar test strips (for diabetes), incontinence products (catheters, disposable undergarments), bandages and protective gloves.
Funding Assistance
Review of funding sources to off set costs of in home modifications.
Technology Assistance
General help with computers, tablets, phones and other technology.
